UK North Fork Valley Community Health Center is a joint venture between the University of Kentucky College of Medicine and the community health board. It is the first community health center in Kentucky to be affiliated with a university and family medicine residency training program and is located in Hazard, KY, at the UK Center for Excellence in Rural Health-Hazard.
UK North Fork Valley Community Health Center provides many types of service and offers a sliding fee scale to help reduce the financial burden that health care costs can create. All patients are treated regardless of their income or ability to pay.
The Center is pleased to offer integrated and comprehensive access to quality Primary Care health services. Our Primary Medical care services are provided in collaboration with our Dental and Behavioral health services at our clinic locations by qualified Family Practice care providers offering the full range of medical health services from newborn well child care, maternal prenatal care, on through to comprehensive geriatric care.
